Explanation of Application Procedures for Various Forms

1. Faculty & Staff Official Email, Non-Official Email, Non-Official Email Password Reset, and Retired Staff Email Retention

Electronic Forms Online Approval System → Computer Center → Information Security & Personal Data (IMS) Zone → Information System Access Rights Application Form → Fill out and submit the application.

2. Password Reset for Faculty, Staff, and Student Emails

Please go to the University Administrative Account Password Reset System to reset your password.

3. Domain Name System (DNS) and SSL Certificate Application (Annual renewal required)

Electronic Forms Online Approval System → Computer Center → Information Security & Personal Data (IMS) Zone → Campus Domain Name System (DNS) & SSL Certificate Application Form → Fill out and submit the application.

4. Wireless Network Authentication Account for Conferences and Events

Please submit an official internal memo (chian-wen) routed to the Network Division.

5. Campus Cloud Virtual Machine (VM) Rental Application (Annual renewal required)

  1. For official or administrative use: Please submit an official internal memo to apply for free usage first.

  2. For project-based systems/websites, laboratory webpages, and faculty personal webpages: Payment is required prior to application.

  3. Procedure: Download the Campus Cloud Virtual Machine Rental Application Form (IMS-P-013-05) from the Information Security & Personal Data Management System (IMS) guidelines → Fill out the form → Obtain the stamp/signature of your department head → Submit the hard copy along with a copy of the approved internal memo to the Network Division of the Computer Center → Wait for a hard copy reply or email confirmation → Completed.

6. Firewall Service Application (Annual re-application required)

In accordance with Ministry of Education directives, the firewall operates under the policy of "Prohibited by Default, Allowed by Exception." Therefore, applications are required for external connections accessing internal campus services.

7. SSL-VPN Application (Annual re-application required per academic year)

University Administrative e-Information Platform → Personal Information & Settings → SSL-VPN Connection Application → Fill out and submit the application.
